SUMMARY
The clinical mentor must take an active role working closely with the mentee to provide guidance, training, support, help define a strong career development plan, and serve as a role model for the mentee. Under the direct supervision of the sub–County AIDS/STI Coordinator and the Fahari ya Jamii program officer; the Clinical Mentor will provide direct technical support to the supported Health Facilities and is responsible for on-site mentoring and implementation of HIV Prevention, Care and Treatment activities. He/she will provide technical advice and support to health care workers in implementation of HIV services (care and treatment, TB/HIV, and PMTCT) to ensure that patients receive the full complement of prevention, treatment and care in accordance to GoK/MoH and other applicable standards. They will provide 100% level of effort.

Specific Job Responsibilities:
1. Provide onsite clinical mentorship on pediatric, adolescent and adult HIV care and treatment to targeted HCWs in collaboration with the sub county mentors
2. Assist in the development of initial and continued medical education for all clinic staff, including facility-based CMEs, OJTs, formal didactic training, and clinical mentorship.
3. Act as a member of S/County HIV Clinical TWG Secretariat to coordinate with sub county mentors, and SCASCOs and organize TWG meetings giving status report to both the County and Regional Clinical TWG.
4. Providing technical support to health workers (doctors, clinical officers, nurses, counsellors, laboratory and pharmacy staff) in all aspects of implementation of HIV services (care and treatment, TB/HIV, GBV and PMTCT) to ensure that patients receive the full complement of prevention, treatment and care in accordance to National HIV treatment guidelines and other applicable standards
5. Coordinate receipt of responses for cases from the clinical TWG consultants and NAHCC graduates and sharing with respective clinicians. Follow up with the clinicians to ensure that the final recommendations are followed and implemented. Provide final report to the clinical TWG on resolved cases.
6. Ensure timely clinical and laboratory monitoring of patients on treatment through the implementation of the viral load standard operating procedure and that timely interventions are undertaken for patients with suspected treatment failure.
7. Building the skills and knowledge of targeted clinical staff on HIV/AIDS prevention, care and treatment, and PMTCT through on-the-job mentorship, case reviews, CMEs and participation in the multi-disciplinary team meetings, County and TWG meetings
8. Mentor frontline health care workers to appropriately document service delivered to clients in the requisite NASCOP/MOH recommended tools.
9. Champion the differentiated service delivery model of care in USAID Fahari ya Jamii supported facilities
10. Support the implementation of both proactive and reactive continuity in treatment strategies and ensure that patients on care are retained.
11. Participating in joint supportive supervision of USAID Fahari ya Jamii-supported HIV prevention, care and treatment activities in assigned sites with County and Sub-County level MoH staff
12. Assist in program monitoring and evaluation with reporting through national HMIS tools and other tools in use by Fahari ya Jamii and its funding partners through mentorship of facility staff on proper documentation.
13. Routinely utilize QI approaches as well as elements of operational research including identification and documentation of ‘best practices’ to ensure provision of high-quality TB/HIV services.
14. Oversee stringent follow-up care of PMTCT clients and HIV exposed infants in assigned sites and ensure that HIV exposed infant exit from the program as healthy infants.
15. Timely preparation and submission of activity budgets, work plans, reports and other technical papers
16. Laisse with Facility clinicians and Adherence Support counselors on pediatric/adolescent case management
17. Perform any other duties as may be assigned by the Fahari ya Jamii Program officers.
18. Assess, provide constructive feedback and document the mentee’s clinical progress and performance.
19. Provide technical support for HIV care and treatment retention, and adherence services for the different age specific cohorts and scale up viral load monitoring to ensure viral suppression of >95%
20. Maintain a constructive dialogue and technical communication with facility staff (peer educators, TPAs and OTZ champs)
21. In collaboration with the M&E team supports program monitoring to ensure data is used to inform programming as needed to contribute to high-quality implementation of HIV/PMTCT/TB/GBV programming

Required Skills and Experience
Education:
• Diploma in Clinical Medicine and Surgery or Nursing from a recognized institution.
• Hold a valid registration certificate from the Clinical Officers Council or the Nursing Council.
Experience:
• Minimum 3 years of practical clinical experience in HIV/AIDS treatment and care at the CCC
• Clinical mentorship experience under NASCOP mentorship Framework
• Experience in pediatric and adolescent ART is strongly preferred
• Trainee Graduates of NAHCC or NIHC have a distinct advantage 95% 20. Maintain a constructive dialogue and technical communication with facility staff (peer educators, TPAs and OTZ champs) 21.
